Many people have contributed to this book. Peter Salus' suggestion first got me thinking about writing it, and Peter put me in touch with Addison-Wesley. Midway through the writing, Blaine Burnham reviewed the completed portions and the proposed topics, and suggested that they be reorganized in several ways.
